The Helmke Library accepts for reserve legal videorecordings only. The videorecording you have submitted for reserve appears to be a duplicate rather than a commercial purchased videorecording. In order to place this tape on reserve you must certify the legality of the recording. Please fill out the information below and submit a copy of this form with each duplicate videorecording you place on reserve. This form will be kept on file at the Service Desk while the videorecording is on reserve and returned to you with the videorecording when it is removed from reserve.
